1. A letter from a medical professional stating the requestee has a permanent life-long disability that precludes them from safely accessing the dock.

2. Must own interest in a dock or have slip(s) on Beaver Lake.

Requestee’s must meet all requirements to be considered for an AAV permit. AAV’s will not be authorized within Protected Shoreline Areas (PDA’s) and Environmentally Sensitive Areas (ESA’s).
