Thank you for using the SWL Vaccination Self-Attest Form. Based on your vaccination status, please see the information below:
If you are fully vaccinated
First off, thank you for getting vaccinated. Vaccination ensures the safety of you, your friends, family, and coworkers. Please be prepared to show your vaccination card or proof of vaccination to your supervisor at a later date.
If you are vaccinated, but not yet considered fully vaccinated
Please work with your supervisor to determine when you will achieve fully vaccinated status. Until you are fully vaccinated, you may be subject to additional testing and possible restrictions to your official travel. Your supervisor will have more information soon.
If you have NOT received any vaccinations
Please consider vaccination. The DOD has determined that all personnel that are not vaccinated will be subject to frequent testing to ensure they are free of infection from COVID-19 or one of its variants. Unvaccinated personnel may also have their official travel restricted or cancelled. Please work with your supervisor to determine how testing will take place. If you choose to get vaccinated at a later date, please let your supervisor know if your vaccination status changes.
If you are unsure of your vaccination status
If you are unsure of your vaccination status, or have complications associated with vaccination, please see your supervisor for more information. Until your status is determined, you will be subject to frequent testing and may have your official travel restricted or suspended. Please work with your supervisor and a medical professional to determine your vaccination status.
If you have a medical exemption for vaccination against COVID-19
If you have an exemption from vaccination against COVID-19 due to an adverse response from the vaccination or other medical complication, please work with your supervisor to ensure they understand the context involved. Please understand that you may be subject to frequent future testing to ensure you are infection free, and you may have your official travel delayed or suspended.
If you are not vaccinated and do not plan to be vaccinated due to personal reasons
Vaccination against COVID-19 is currently a personal choice. That said, vaccination is strongly recommended by the DOD and CDC both. All federal employees are required to be vaccinated or to undergo frequent testing to ensure that they are infection free. This testing is required to ensure your safety as well as that of your coworkers. Please consider vaccination and work with your supervisor to ensure your status is documented. As above, unvaccinated employees may have their official travel delayed or suspended until they are fully vaccinated.
